Lip Filler in London: Natural Lip Fillers, Russian Lips and Lip Enhancement

The lip is a muscle wrapped in the thinnest skin on the face, with an artery running through it. Good lip filler respects all three: it follows the shape you already have, it is placed where the anatomy allows, and it stops before anyone can tell. I have trained other doctors and nurses in this for a decade, and I would still rather under-fill and review than chase a look in one visit.

Lip filler is a hyaluronic acid gel injected into the lips to restore or add volume, define the border and Cupid’s bow, correct asymmetry, hydrate dry, lined lips or lift the corners of the mouth. It takes 30 to 45 minutes, the filler contains a local anaesthetic, and the result is visible immediately and settles over two weeks as swelling clears. Most first-time patients need 0.5 to 1 ml; the lips break filler down faster than the rest of the face, so a result lasts six to twelve months. It is fully reversible with hyaluronidase. I use Juvéderm, Restylane and Stylage, chosen for the softness or structure each lip needs, and I place filler by cannula or fine needle in the plane above the labial artery, which is the single decision that keeps the treatment safe. Lip filler at Hampstead Aesthetics costs £220 for 0.5 ml, £300 for 1 ml and £400 for the Russian lip technique, with the consultation and the two-week review included.

ABOUT LIP FILLER

What lip filler is, and the anatomy that decides how it is done

The lip has more structure than it looks. The red part, the vermilion, is bounded by the white roll, a faint ridge of skin that catches the light and gives the lip its edge; above it the two philtral columns run up to the nose, and the Cupid’s bow sits between them. The upper lip has three small tubercles of fat, the lower two, which is why a natural lip is not a smooth tube. Underneath is the orbicularis oris, the muscle that purses, and running through the muscle near the wet-dry border is the labial artery, with its branches to the philtrum and the columella. The whole thing is covered by skin so thin that anything placed too shallow shows as a bump and anything placed in the wrong plane can be seen or, in the case of the artery, can block it.

Hyaluronic acid filler is a gel of the same sugar molecule the skin already contains, cross-linked so that it holds its shape for months. Fillers differ in how firm they are and how much they swell: Restylane Kysse and Juvéderm Volbella are soft and flexible, made for lips that move; Juvéderm Ultra and Stylage Special Lips are a little firmer and hold definition; a firmer gel again is occasionally used to support a very thin lip. Every one of them contains lidocaine, so the lip numbs from the first injection, and every one of them dissolves within a day if hyaluronidase is injected into it, which is what makes lip filler the most forgiving of all the injectables.

What lip filler can do: restore volume that has thinned with age, define a blurred border, balance an upper lip that is smaller than the lower, lift the corners of the mouth, smooth the fine vertical lines that lipstick bleeds into, and hydrate lips that are always dry. What it cannot do: it cannot make a small mouth wide, it cannot lift a long upper lip, which is the job of a lip flip or surgery, and it cannot give a natural result if the volume asked for is more than the lip can hold, which is where duck lips, the moustache shadow of migrated filler and the flat, ledge-shaped upper lip all come from. The dermal fillers page explains where the lips sit in a wider facial plan, and my blog on lip filler facts covers the medicine in more depth.

THE TECHNIQUES

Five ways I treat lips, and who each is for

The consultation is about which of these you need, not how many millilitres you can have.

Natural definition

0.5 to 1 ml placed along the border and the tubercles. Hydration, a crisper edge and a little volume that nobody can identify. The first treatment for most patients.

Russian lip technique

Small vertical threads from the wet border outwards, which lift and heighten the lip and keep the profile flat. A fuller lip from the front without projection. £400 for 1 ml.

Balance and asymmetry

Correcting an upper lip that is smaller than the lower, a lopsided Cupid's bow or an uneven lip after previous filler. Often 0.5 ml placed with intent.

Lip lines and hydration

A soft gel placed superficially to smooth vertical lip lines and the dryness that comes with age, without adding volume. Often combined with skin boosters.

Correction and dissolving

Migrated or lumpy filler from elsewhere dissolved with hyaluronidase, then the lips rebuilt from two weeks later. £300 to dissolve.

Lip flip

Not a filler: a few units of anti-wrinkle injection at the lip border to roll the upper lip outward. Subtle, three months, £150, and a good test for someone unsure about filler.

THE TREATMENT

What happens at the consultation and the treatment

The consultation takes 15 minutes at the start of the appointment, longer if you are new to filler. I look at the lips at rest, smiling and speaking, and at their relationship to the chin, the nose and the teeth, because a lip that is right for one face is wrong for another. The classical proportion is an upper lip about two thirds the height of the lower, and a lip that projects a little less than the nose and a little more than the chin in profile; a receding chin makes lips look bigger than they are, which is why some patients who ask for lip filler leave with a recommendation for chin filler instead. I ask what you want in your own words, look at any photographs you have brought, and tell you honestly what the volume you have in mind will look like. Then we agree the product, the volume and the price in writing.

The treatment takes 30 minutes. A prescription-strength numbing cream goes on for 15 to 20 minutes; the filler itself contains lidocaine, so the lip numbs further after the first injection. I place the filler with a fine needle along the border and the body of the lip, or with a blunt cannula from a single entry point at each corner, which reduces bruising and keeps the tip away from the artery. The technique follows the plan: linear threads along the border for definition, small boluses at the tubercles for shape, vertical threads for the Russian lip, and a superficial fan of a soft gel for lines. I inject slowly, in small amounts, and check the colour and capillary refill of the lip after each pass. You see the shape immediately, a little exaggerated by swelling.

You leave with written aftercare and my direct number. The review is at two weeks, when the swelling has gone and the true result can be judged; if a touch of filler is needed to balance, it is added then. Lip filler combines with chin filler for the profile, with nasolabial fold and marionette line filler for the lower face, and with anti-wrinkle injections at the mouth corners or as a lip flip. Where the lips are lined and dry rather than thin, a course of skin boosters does more than volume.

“I would rather under-fill and review than chase a look in one visit. Lips swell, and judging the final shape takes a fortnight. Building gradually is how you get a natural result that still looks like your mouth.”

SWELLING AND SETTLING

Lip filler day by day

Lips swell more than any other area. This is what to expect, and when to worry.

Time What is happening What you see What to do
Day of treatment
Anaesthetic, filler and injection trauma
Lips larger than the final result, sometimes uneven; pinpoint marks
Cold pack for ten minutes at a time; no lipstick for 12 hours; sleep with an extra pillow
Days 1 to 2
Peak swelling
Lips firm, full, occasionally lopsided; bruising appears if it is going to
Arnica if you like it; no exercise, alcohol, saunas or hot drinks; do not press or massage
Days 3 to 5
Swelling receding; filler integrating
Lips soften; small lumps along the border are normal and settling
Normal life; gentle lip balm; avoid dental work
Day 7
Most swelling gone
Shape close to final; bruising yellowing
Lipstick, exercise and everything else back to normal
Day 14
Filler fully settled
The true result
Review appointment; any balancing touch added now
Months 6 to 12
Hyaluronic acid gradually metabolised
Lips slowly returning to their previous shape
Top-up when you and I agree it is due, typically 0.5 ml

Call me the same day if a patch of lip turns white, dusky or blotchy, or is far more painful than the rest; that is the sign of a blocked vessel and it is treated immediately with hyaluronidase.

RESULTS

Results and what to expect

A natural lip filler result is one that people notice as you looking well rather than as a procedure: a crisper border, a lip that catches the light, a balance between upper and lower that was not there before, and lipstick that stays where it is put. Half a millilitre changes definition more than size; one millilitre adds visible fullness across both lips; anything beyond that in one visit is a decision about the look you want rather than what the lips need, and I will say so. The Russian technique gives height and a heart shape from the front with a flat profile, and suits a lip that is short rather than thin. Lines and dryness respond to a soft, superficial gel or a skin booster more than to volume.

The honest expectations. Lips are the one area where over-treatment is common enough to have a vocabulary: duck lips from filler pushed forward, the ledge or shelf from filler placed too high, and the moustache shadow from filler that has migrated above the vermilion border, which happens when too much is placed too shallow or too often. All three are avoidable with sensible volumes and correct placement, and all three are reversible with hyaluronidase. Swelling makes every lip look overfilled for two or three days. Results last six to twelve months and fade gradually; filler does not stretch the lips, and if you stop, they return to how they were.

I photograph before, immediately after and at the two-week review, in the same light.

AFTER TREATMENT

Recovery, downtime and aftercare

The first 12 hours. No lipstick, lip liner or balm other than the one I give you. Cold packs for ten minutes at a time. No touching, pressing or pouting at the mirror. Sleep on your back with an extra pillow.

The first 48 hours. No exercise, saunas, steam rooms, hot yoga, sunbeds or alcohol, all of which increase swelling and bruising. Avoid very hot drinks and salty food. Kissing is best left for a couple of days.

The first two weeks. No dental work or facials. No flights within 48 hours if it can be avoided. Lumps along the border are normal while settling; do not massage them unless I have shown you how.

Bruising. Possible at any injection point, more likely with a needle than a cannula and in patients on aspirin, fish oil or ibuprofen. Arnica helps some people. Bruises can be covered with make-up from the next day and fade within a week.

You leave with written aftercare, the date of your two-week review and my direct contact details for any concern.

IS THIS FOR ME?

Is lip filler right for you?

Most adults who want it are suitable. The consultation is about the right volume and technique, and occasionally about a different treatment altogether.

A good candidate

Naturally thin lips. Lips that have thinned or lost their border with age. An upper lip smaller than the lower. Asymmetry. Vertical lip lines and lipstick bleed. Dry, crepey lips. A lip that disappears when you smile. Previous filler elsewhere that you want corrected or refined. Any skin type and tone.

Better with something first or alongside

A receding chin: chin filler first, then lips if still wanted. A long upper lip: a lip flip or a surgical lip lift rather than volume. Marionette lines pulling the corners down: mouth-corner filler or anti-wrinkle injections alongside. Migrated or lumpy old filler: dissolve, wait two weeks, then rebuild.

Not suitable now

Pregnancy or breastfeeding. An active cold sore, or any infection around the mouth. Dental work in the last two weeks or the next two. Certain autoimmune conditions and a history of serious filler reaction, discussed at consultation. Anyone wanting a volume I believe will look unnatural, whom I will decline honestly.

WHY US

Why choose Hampstead Aesthetics for lip filler

Lips are the injectable most often done badly, because they are the one most often done by someone who has learned a technique without learning the anatomy. I am a GMC-registered doctor (4418489), a Member of the Royal College of Surgeons of Edinburgh, a GP with a master’s degree in aesthetic medicine, a senior trainer for Derma Medical and a faculty trainer for Stylage, which means I teach other doctors and nurses to inject lips. Every consultation, every injection and every review is done by me, and I am the person who answers the phone if something worries you afterwards.

I stock Juvéderm, Restylane and Stylage so that the gel matches the lip rather than the other way round; hyaluronidase is on the shelf for every lip appointment, not ordered in. The clinic is on West End Lane in West Hampstead, NW6, two minutes from West Hampstead Jubilee line, Overground and Thameslink stations, and sees patients from Hampstead, Swiss Cottage, St John’s Wood, Belsize Park, Kilburn, Primrose Hill, Maida Vale and across North and West London.

AT HOME

What makes lip filler last and look natural

A lip balm with SPF every day, because ultraviolet breaks down hyaluronic acid and the lips have no melanin to protect them. No smoking, which halves the life of every filler and deepens the lines above the lip. Stay hydrated; the filler holds water and dehydration shows in the lips within a day. Do not pick, bite or chew the lips. A top-up of 0.5 ml at six to nine months keeps the result without ever needing a large volume again, and over the years most patients find they need less rather than more. Where the skin above the lip is lined, a retinoid through the prescription retinoid programme and a course of skin boosters do more than filler can.

DOCTOR LED TREATMENT

Why Doctor Led Matters

The superior and inferior labial arteries run through the lip, in most people between the muscle and the wet border, and in a proportion of people closer to the surface than the textbooks say. Filler injected into one blocks it: the lip blanches, then darkens, and without hyaluronidase within hours the tissue dies. It is the commonest serious filler complication in the UK, and the lips are where it happens. The UK has no legal requirement for the person injecting lips to hold any qualification, and lips are the treatment most often given by non-medical practitioners after a short course.

At Hampstead Aesthetics I assess the lips, choose the product, perform every injection myself, check the colour of the lip after every pass, and keep hyaluronidase, an anaphylaxis kit and a written occlusion protocol on the shelf. I hold the ACE complication management certification and every patient has my direct number.

YOUR SAFETY

FAQ's

Frequently Asked Questions

How much does lip filler cost in London?

At Hampstead Aesthetics 0.5 ml is £220, 1 ml is £300 and the Russian lip technique is £400 for 1 ml, with the consultation, a premium filler and the two-week review included. A lip flip is £150 and dissolving unwanted filler is £300. Across London, 1 ml by a doctor is typically £350 to £600.

Six to twelve months for 1 ml and four to six for 0.5 ml. The lips move constantly and have a rich blood supply, so they break filler down faster than the cheeks or chin. Most patients top up with 0.5 ml at six to nine months, and many find the interval lengthens over time. My blog on how long lip filler lasts covers what shortens and lengthens it.

Less than people expect. A prescription numbing cream is applied for 15 to 20 minutes and the filler itself contains lidocaine, so the lip numbs further after the first injection. Patients describe a pinch and pressure rather than pain, and a cannula reduces this further. The lips are tender for a day or two afterwards.

Most first-time patients need 0.5 ml for definition or 1 ml for visible fullness. I decide at consultation from the size of the lips, the balance between upper and lower, the chin and the look you want, and I would rather place 1 ml well and add 0.5 ml at the review than place 2 ml in one visit. Anyone who is told they need 2 ml or more as a first treatment should ask why.

A technique in which small vertical threads of filler are placed from the wet border towards the edge of the lip, rather than along its length. It lifts and heightens the lip, opens the Cupid’s bow and keeps the profile flat, so the lips look fuller from the front without projecting forward. It suits a lip that is short rather than thin, swells more than the classic technique and lasts a little less. I charge £400 for 1 ml because it takes longer and more precision.

Yes, if the volume is sensible and the placement is right. Unnatural lips come from too much filler, filler placed too shallow or too high, or a shape imposed on a face it does not suit. I plan to the proportions of your face, place the filler where the anatomy holds it, stop at a volume the lip can carry, and review at two weeks. The photographs on this page are the result I aim for.

Yes. Hyaluronic acid filler dissolves within 24 to 48 hours when hyaluronidase is injected into it, whether that is to correct a lump, to remove migrated filler or because you have changed your mind. I keep it on the shelf for every appointment and it costs £300 as a treatment in its own right. My dissolving page explains the process.

Migration is filler that has spread above the vermilion border into the skin of the upper lip, giving a flat shelf or a shadow like a faint moustache. It happens when too much filler is placed too superficially or too often, and it is more common with repeated top-ups elsewhere. It is fixed by dissolving the migrated filler with hyaluronidase, waiting two weeks for the lips to return to their own shape, and rebuilding conservatively. Around a fifth of my new lip patients arrive with migration from previous treatment.

Filler adds volume and definition and lasts six to twelve months. A lip flip is a few units of anti-wrinkle injection at the border of the upper lip, which relaxes the muscle so the lip rolls slightly outward; it adds no volume, lasts two to three months, costs £150, and suits someone whose upper lip disappears when they smile or who wants to try a subtle change before filler. The two combine well.

Peak swelling is at 24 to 48 hours, when the lips look larger and sometimes uneven; most of it has gone by day five to seven, and the final result is at two weeks. The Russian technique swells more and for longer. Cold packs, sleeping propped up and avoiding heat, alcohol and exercise for 48 hours all help. Book at least two weeks before any event.

Yes, with a different approach from volume. A soft, superficial gel smooths the vertical lines above and in the lip and stops lipstick bleeding, without making the lip bigger. For finer lines and dryness a course of skin boosters into the lips and the skin above them works well, and anti-wrinkle injections in small doses soften the lines that come from pursing. Deep lines above the lip respond to laser resurfacing.

Asymmetry is the commonest reason for a small, targeted treatment. Almost every lip is slightly uneven at rest, and more so on smiling; 0.3 to 0.5 ml placed precisely on the smaller side, or at one corner, balances it. Where the asymmetry comes from previous filler, that is dissolved first.

Usually, yes, with a plan. Any injection to the lip can trigger an outbreak in someone prone to them, so I prescribe antiviral cover to start the day before treatment. Treatment is postponed while a cold sore is active and until it has fully healed.

Not at sensible volumes. The lips are elastic, and when hyaluronic acid filler wears off they return to how they were. Very large volumes placed repeatedly over years can stretch the tissue, which is one of the reasons I decline them. Half a millilitre every six to nine months does not.

The one that suits your lips. Restylane Kysse and Juvéderm Volbella are soft and move naturally, and are my choice for definition and hydration; Juvéderm Ultra and Stylage Special Lips hold shape better and suit a lip that needs structure. I stock all three families and choose at consultation, and I train other injectors in the Stylage range. What matters more than the brand is that it is a CE-marked hyaluronic acid product from an authorised distributor, which every filler I use is.

The same day. There is no medical downtime; the social downtime is the swelling and any bruising for two or three days, which make-up covers from the next morning. Avoid exercise, alcohol and heat for 48 hours and dental work for two weeks.
